Like many have posted here, I used my initials - DEF.
This comes in real handy on systems that would start the next letter in the same place were you put the first one.
One these game (which was most of them), I would go to D, then just nudge the controller slightly to E, and then F. I loved doing that!
(For systems that allows more than the measly three characters, it would be OZYR, of course)
Of course, as with others here, I would put in 'other' things now and then.

SUX (for bad games)
and other such things
That is the stuff I usually put into MAME scores now on the computer, as I'm tired of looking at my initials all the time!

I'm another one that just uses initials.
I think it's slightly funny though because it spells out
VAG
Take that as you will. I think I like my brother's initials better though: ARG. (His A is for Andrew, my A is for something else)
As for games that allow a full name I'll just put my normal name in: Vance.
Although if I get frustrated at the name entry it just ends up being junk. (IE Le Mans 24 one time I just ended up with I think UVV because I kept missing the V and I down to like 3 seconds)

When I first started playing, I would use my initials of RLS but after awhile just went with ROG, a shortened version of my first name. I never got very creative and never even thought of using profanity...that's just not me, I guess...
It depends on the length really. If I can use enough letters I will use Pieter or Speedy, since they are my first name and nickname, respectively. When only 3 letters are allowed, I'll usually put down PIE since nobody else ever seems to use that, so I can differentiate my high-scores from the rest. If I spend a lot of time behind a machine where you can enter lots of letters I also sometimes try and write up a story/phrases using high-scores
