Sega Swirl

I picked up a copy of House of the Dead 2 for PC the other day, and of course it had a free copy of Sega Swirl with it. Seems to me like 3-4 years ago Sega was milking the hell out of this game! Everywhere you turn...oh, its on this demo, oh, its on this game, oh wait, you can download free here!

Thing is, I'd like to see a list of all the disk and places this game was made available at. Just out of sheer curiosity. If you have a disk with this game on it, please post it!

There's also a freaking Palm OS version, and it's on the Sega Smash Pack for Dreamcast.
