Another Smash TV wouldnt hurt either.
Closest you're probably going to get to another Smash TV is CrimsonLand. I'd link to it, but it used to be freeware. (Note used to be.) Nice game though. At least it used to be. Can't say.

Only game to put me in game-mode so bad that I was suspiciously eyeing text for a half hour afterwards thinking it was about to kill some spot inbetween the lines :P

Default No contest .... HERZOG ZWEI!

This was the best game of the 16-bit era and should definitely have a sequel!

New stuff I'd like to see:

    Bubble Bobble in 3D (platformer?)
Jackal (from NES...with jeeps and guns...remember?)
Tomba! (from PS1...great character, where'd he go?)
    Kid Icarus (yeah, baby)
Rad Racer (the NES equivalent of Out Run, which is getting a sequel)
Hang-On (as a competitor to MotoGP, done in Sega GT style)
Golden Axe (maybe done in Gauntlet style)
Adventure (perhaps in 3D with the original map and restrictions...and the easter eggs!)
Zillion (if they did a new Metroid, they can certainly do a new Zillion)

    And finally...

Alex Kidd, for God's sake! He dropped off the face of the earth once Sonic arrived...it's time we saw his big ears again!
Slowly rebuilding the gaming rig in my new apartment. Gotta use the HDTV for something...
Systems owned: INTV2, NES, SMS, Genesis, Lynx, SNES, Saturn, PS1, GBC, DC, PS2, GBA, Xbox, GC, PSP, DS, Wii
    Systems wanted: 2600, Coleco, TG16, GG, X360

You mean the one mentioned in the Commander Keen Manual? That eventually became Duke Nukem (the first one).
    No...

Duke Nukem and Quake were developed by Apogee Software and iD Software, respectively. I'm not 100% on my id/Apogee history, so I can't say if these separate software houses had split up when this was released, but I'm guessing they already had.

No, (I am working from memory here, but it's a solid memory) Quake, as described at the end of Keen (IV, I think) was some sort of big, bad fellow who started out supercharged (rather unlike the final form of your character) with magical artifacts...Hammer of Thor was one, I believe. They changed the others around a bit for the real Quake, but I have the feeling they mentioned a Ring of Regeneration and an Amulet of Shadows (somewhat different from the final product again, of course).

I can't comment on whether it was seen in the manual (my guess is no, not likely) but it was certainly seen at the end of one of the Keen games.

I tried to rediscover the website that lists this (along with some other interesting DOOM related alphas/betas and the interesting OS/2 wallpaper that the mountain background in DOOM was taken from) was on, but wasn't able to discover it. I do hope it hasn't been taken down after all these years... :/
You see, at the time Commander Keen was made, Apogee and id had some sort of an alliance and released games made by both companies (like the Keen games, and Wolfenstein 3D). It was because Tom Hall, John Carmack, Adrian Carmack and John Romero were working at Softdisk during the day. JC developed some sort of scrolling background (regular for consoles but revolutionary for PC), and the four started working on the first Commander Keen game. Scott Miller (owner of Apogee) was impressed by Romero's early games and drew the four of them into making the game for Apogee. Shortly after its released, they formed id software. In a sort of in-game manual, there was a mention of Quake exactly as you described. Here's the text, recreated word-for-word:

"As our follow-up to the Commander Keen trilogy, id Software is working on 'The Fight for Justice', a completely new approach to fantasy gaming. You start not as a weakling with no food--you start as Quake, the strongest, most dangerous person on the continent. You start off with a Hammer of Thunderbolts, a Ring of Regeneration, and a trans-dimensional artifact. Here the fun begins. You fight for Justice, a secret organization devoted to vanquishing evil from the land! This is role-playing excitement.

And you don't chunk around the screen. 'The Fight for Justice' contains fully animated scrolling backgrounds. All the people you meet have their own lives, personalities, and objectives. A 256-color VGA version will be available (smooth scrolling 256-color screens --fancy that)!

And the depth of play will be intense. No more "whack whack here's some gold." There will be interesting puzzles and decisions won't be "yes/no" but complex correlations of people and events. 'The Fight for Justice' will be the finest PC game yet."

I guess that parts of it were used in Duke Nukem, while other parts where used in Quake.
Whoever wins, the battle doesn't end. The loser is freed from the battlefield, but the winner must remain there, and the survivor must live his life as the warrior until he dies.

Well I say Atomic Runner is long overdue for a sequel. Why it still hasn't got one is beyond me. And I second Demon's Crest. That would be sweet also.

I can say with pride that Beat 'um and Eat 'um deserves a sequel. Oh yeah and so does Wally Bear and the No! Gang.

I wish they brought the Langrisser sequels over here...they would make a great addition to the GBA. I'm playing the translated Langrisser 2 ROM now and loving it (just like Warsong...PM me if you want me to send it to you).

I would like to play another Einhander game. Although a small part of me feels it should be left alone and that not everything needs to be turned into a sequel.

I wish there had been a sequel to Killing Time for the 3DO, Outlaws and Full Throttle for PC.

It depends on how things are done. a newer sequel to Kid Ikarus would rock! but other than that:

    Mega Man(like the NES versions)
Mario Bros(yes i'm aware how many Mario games there are)
    Yet another Tecmo Superbowl(IV)
    Donkey Kong(4)

all 2D, just like the NES versions. As you can see, I love NES

And sequels I would like not to see:

All this new crap that has nothing to do with the previous game just so the game is a shoe in. I hate buying a sequel and it is NOTHING like any of the previous ones, and it sucks on top of that.

I'd like to see a new version of Duck Hunt. It would have to be like Doom and you could be the hunter and run around with various guns and weapons. Once you beat the game with the hunter, you could play again with the dog. There could also be a more expensive version boxed with a light gun and you would be able to play a revamped version of the original.

I'd like to see another Nights Game, another Panzer Dragoon Saga game. How about a shining force as well as a true phantasy star game. Dynamite heady and Ristar would be cool
